Switch GrEffect::onIsEqual signature back to GrEffect from GrEffectRef.

All the changes outside of GrEffect.h are just removing "Ref" from every
subclass's onIsEqual signature and changing two instances of "->texture(0)" to
".texture(0)" in onIsEqual() impls.
