Descriptionmarkup.scm: space a line by extents, not refpoints
Now functions like \parenthesize keep the refpoint of their argument,
and be properly spaced in a line. The horizontal component of
\translate no longer has visible effect in a line markup, but \hspace
remains useful for this function.
cleanup some redundant code found by dak
