Description

Build everything on first pass of make doc. Currently if one does a make doc from scratch and then immediately do another make doc, a bunch of stuff is being rebuilt. This patch addresses this. - Remove a couple of unused rules. - Explicit dependency on generated version.itexi. - Fix Documentation/css, ly-examples, pictures, topdocs.
